SUMMARY:
The State of Alaska Department of Health, Division of Public Health seeks a vendor to conduct a statewide community health needs assessment to identify communities across the state experiencing the greatest unmet health care needs, outcome disparities, and workforce shortages; poverty; and barriers to accessing care. Contractor will coordinate regularly with the project director for work phase advancement approval and will combine findings into a professional report for dissemination amongst stakeholders.
