Question:  "The RFP lists specifically what must be included in the proposal, the format and everything that must be included.  It does not include any of the IT related forms, one of which is titled "Required Vendor Response".  How or when would that be submitted?"

Answer:  The IT-related forms are referenced in the RFP on page 47 of 47 under the heading "Attachment 4.A IT-Related Forms" which are attached as separate documents to this OPN solicitation posting as "Attachment 4.A - <IT Form Name>", etc.

The submission of form "Attachment 4.A - Required Vendor Response - DOH IT Requirements (ver 20220824).xlsx" is non-scored but should be included at time of proposal submission.

NOTE:  If offeror anticipates the above clarification has potential impact on submitted proposal, the offeror may request to retract pertinent portions or all, to endeavor to make any necessary adjustments and resubmit proposal before the extended deadline to accommodate this clarification, as noted in Amendment 3 (extended to 2/18/23, 2:30PM AKST).

SUMMARY:  The Division of Public Health, Section of Women's, Children's and Family Health (WCFH), is soliciting proposals to contract with a relational database provider for HIPPA compliant clinical, programmatic, and financial information for the State of Alaska's Breast and Cervical Cancer Screening Program.  The Program (Alaska Breast + Cervical Screening Assistance Program - AK B+C), located in WCFH, is established under federal law and funded by the Centers for Disease Control and Prevention (CDC) under the National Breast and Cervical Cancer Early Detection Program (NBCCEDP).  AK B+C has been screening low income Alaskans since 1995.
