Amendment # 1
This is a mandatory return Amendment. The purpose of this Amendment # 1 is to: The Department of Transportation and Public Facilities (DOT&PF) is soliciting proposals to result in a single multi-year contract for the following services:
1. Acompany that is accredited conforming to ANSI/ISO/IEC Standard 17025:2005,General Criteria for the Competence of Testing and Calibration Laboratories.
2. FurnishUSB and TLD type radiation detection badges including data support to analyze,and summarize radiation detection badges and annual accumulation exposurereadings.
3. Calibrate up to 80 Troxler 3400 seriesMoisture-Density gauges and up to 5 Troxler 4590 moisture density gauges peryear on-site (Anchorage, AK).
4. Performnon-routine maintenance on-site for up to 80 Troxler 3400 seriesMoisture-Density gauges, up to 6 Troxler 3241-C asphalt content gauges,and up to 5 Troxler 4590 moisture density gauges per year on-site(Anchorage, AK).
5. Acompany in compliance to ASTM D 7759 Standard Guide for Nuclear SurfaceMoisture and Density Gauge Calibration and have been approved by the NuclearRegulatory Commission (NRC) to provide and analyze leak test for use on StateMoisture-Density gauges.
6. Performout of sequence calibration and non-routine maintenance at the contractor'sfacility as required.
7. Perform an annual audit (if DOT&PFrequests the service) of DOT&PF's radiation safety program at the StateRadiation Safety Office in Anchorage prior to November 15 of each contractyear.
8. Perform Calibration Blocks calibration asrequired (at least every five years).
It is theintent of the state to award one (1) contract in response to thissolicitation. The contractor shall be selected by points earned per theevaluation criteria of this RFP.
Proposalsmust be received no later than 3:00P.M. Alaska Time June 4 , 2020 Emailed,faxed or oral proposals will not be accepted.
