Unusual Occurrence Report (UOR) Tool
The Department of Health and Social Services, Division of Alaska Psychiatric Institute (API), is soliciting proposals for an electronic unusual event reporting (UOR) tool. The tool will help reduce redundancies in API’s current system by bringing the reporting of medication errors, escalating patient behaviors, patient complaints/grievances into one reporting system.

Prior Experience

The offeror must have three (4) years in the last ten (10) of experience constructing UOR tools for Healthcare providers.

Reporting system must have at least five (5) years of experience in the past ten (10) years utilized by other healthcare facilities. Link medical records to the reporting system. Each incident must be emailed to the incident preparer

Reporting system must have at least four (4) years of experience seven (7) years providing history of healthcare facilities that utilized the service. Link medical records to the reporting system
An offeror's failure to meet these minimum prior experience requirements may cause their proposal to be considered non-responsive and their proposal may be rejected.
