This Request for Quotation (RFQ) is intended to result in a contract to provide courier services for the Division of Public Assistance. The length of contract will be from January 1, 2020 through December 31, 2021, with the option to renew for an additional one (1) year period under the same terms and conditions as the original contract. Renewals to be exercised solely by the State. 

This bid will result in a contract to provide daily courier service for the Division of Public Assistance between; Anchorage Alaska Native Medical Center (AMNC), Gamble District Office, Muldoon District Office, Frontier Building, and Mat-Su District Offices.

Quote Submittal: To be considered responsive, the bidder must return the following items:
1. Page 1 of this RFQ with Contractor information completed and signed.
2. Page 5 – Bid Schedule filled out.
3. Page 13 – Federal requirements signed.
4. Product or service offered must meet the specifications as described in this RFQ.
5. Return any amendments that request the amendment be signed by the bidder and returned with the bid.

ALL QUOTES ARE DUE BY: 12/2/19 NLT 4:00PM AKST.
A bidder’s failure to submit its quote prior to the deadline will cause the quote to be disqualified.
Late quotes or amendments will not be opened or accepted for evaluation.
Faxed quotes will not be accepted.
Oral quotes will not be accepted
