The purpose of this amendment is to change Appendix B2 to APPENDIX B1. See attached Appendix B1 below. Appendix B2 should be removed from the solicitation and replaced with the attached Appendix B1.

The State of Alaska, Department of Corrections is requesting proposals for the services of a qualified individual or agency to provide Dental Assistant services in support of inmate health care at the Wildwood Correctional Center, in Kenai, Alaska and the Spring Creek Correctional Center in Seward, AK. Offerors must meet minimum qualifications.
Sealed proposals will be received until 2:00 p.m., August 16, 2018. The services are anticipated to begin approximately October 1, 2018 and continue through June 30, 2019 with optional one year renewals up to June 30, 2022, if exercised.
