Second Request for Agency Participation in Enhanced Care Coordination Study
The Alaska Department of Health and Social Services, Division of Senior and Disabilities Services (SDS) is establishing a two-year enhanced care coordination study to assist in service delivery design of a person-centered structure for home and community-based waiver and state plan services.
Care coordination agencies interested in applying for inclusion in the enhanced care coordination study should review eligibility guidelines linked here:

Agencies wanting to be considered for inclusion in the enhanced care coordination study must submit an application, in the form of a response to the eligibility guidelines, by 5:00 p.m. on September 30, 2016. Earlier submissions will be considered as they are received. Agencies that previously submitted applications do not need to reapply.
