The Department of Health and Social Services, Division of Senior and Disabilities Services, seeks proposals from eligible applicants to provide services through the Traumatic and Acquired Brain Injury Program.

Applicants must create separate applications for each program being applied for. In the Project Name field specify which program application is for, either Part A: TABI Case Management or Part B: TABI Mini Grants.
