ITB 06 170007271 4 https://iris-vss.alaska.gov/webapp/PRDVSS1X1/AltSelfService Amendment two serves to change sections of the ITB & to answer questions submitted by interested parties.The Alaska Dept. of Health & Social Services (DHSS), Div. of Senior and Disabilities (DSD), Governor's Council on Disabilities & Special Education (GCD&SE) is seeking competitive bids to establish a one (1) year contract, with the option to renew for three (3) additional one-year terms to provide real-time captioning & transcription services. Questions must be submitted in writing to the Procurement Officer NLT 2:00 PM Prevailing AK Time on April 5, 2017.Bid Submittal: To be considered responsive, the bidder must return the following items:1. Pages 1-2 of this ITB with the contractor information filled in and signed;2. Page 22 - Bid Schedule completely filled in as requested;3. Page 24 - Federal requirements signed;4. Product or service offered must meet the specifications as described in this ITB;5. Return any amendments that request the amendment be signed by the bidder and returned with bid; and6. The terms and conditions of this ITB must be accepted as written. Any changes made to the terms and conditions and/or specifications within the ITB will disqualify the bid and the bid will be declared non-responsive. 2 AAC 12.840(b) states: "A bid that is conditioned by the bidder and the condition requires a change to a material term of the solicitation is nonresponsive unless conditioned bids are specifically authorized in the invitation to bid.
