NOTICE OF COMMISSIONER'S DECISION TO ISSUE A CERTIFICATE OF NEED

THE DEPARTMENT OF HEALTH AND SOCIAL SERVICES

In accordance with 7 AAC 07.070, the Commissioner of the Department of Health and Social Services has issued a Certificate of Need for the application described below.

Project/Activity: Fairbanks Medical Imaging, LLC d/b/a North Star Radiology proposes construction of a 4,800 square foot facility that includes the addition of a Computed Tomography unit, expansion of Fluoroscopy services, and replacement of a Magnetic Resonance Imaging machine.

Applicant's Name:  Fairbanks Medical Imaging, LLC d/b/a North Star Radiology

Project/Activity Service Area: Fairbanks, Alaska

Summary of Decision: In accordance with A.S. 18.07.031-111 and 7 AAC 07.001-900, Fairbanks Medical Imaging, LLC d/b/a/ North Star Radiology is authorized to build/renovate an independent diagnostic testing facility, as proposed, in Fairbanks, Alaska. The project was completed on July 11, 2011.

This authorization is for construction of a 4,800 square foot facility that includes the addition of a Computed Tomography unit, expansion of Fluoroscopy services, and  replacement  of  a  Magnetic  Resonance Imaging machine.

The total cost of the project is $2,153,392.

The project was completed on July 11, 2011. Per 7 AAC 07.070(h), "Rights granted under a certificate of need do not continue after the activity authorized by the certificate of need is completed."

An applicant may appeal the Commissioner's decision by contacting Alexandria Hicks as indicated above. An appeal must be made in accordance with 7 AAC 07.080 and must be received by the department no later than 4:30pm Monday, April 14, 2014.

Any member of the public who is substantially affected by activities authorized by this certificate of need may initiate an administrative hearing under 7 AAC 07.082. The request must be received by Alexandria Hicks as indicated above, be made  in accordance  with 7 AAC 07.082, and must be received by the department no later than 4:30pm Monday, April 14, 2014.
