Amended RFP - Comprehensive Behavioral Health Treatment and Recovery Program for FY14-FY16

****Amendment #4 issued on 4/23/13 serves to answer questions regarding the Request for Proposals (RFP) submitted by potential applicants on or before the inquiry deadline.(See Amendment #4.)****

****Amendment #3 issued on 4/16/13 serves to answer questions received from potential applicants regarding the Requst for Proposals.  (See Amendment #3.)  ****

****Amendment #2 issued 4/9/13 serves to give a summary of the Pre-Proposal teleconference held on 4/3/13 at 1:30 p.m, answer questions received from potential applicants, amend language in Section 2.02 and to replace Attachments #3, #4 and #5 in their entirety with revised forms.****

**** Amendment #1 issued 4/1/13 serves to amend language in Section 1.03 and 3.05, provide clarification to Section 4.05 criteria, replace Attachments #2 and #5 with revised forms and answer questions received from potential applicants regarding the Request for Proposals (RFP).  (See Amendment #1, as well as Attachment #2 & Attachment #5) ****


The Department of Health and Social Services, Division of Behavioral Health, seeks grant proposals from eligible applicants for Comprehensive Behavioral Health Treatment and Recovery Services for FY 2014.

To be eligible for consideration, proposals must be received no later than 4:00 pm prevailing local time on April 30, 2013.  Faxed, verbal or e-mailed proposals will not be accepted.  Proposals received after the deadline will be considered non-responsive.

A Pre-Proposal Teleconference will be held at 1:30 PM on April 3, 2013.  To participate in the conference call, interested applicants must call 1-800-315-6338 and use code 5808.
