Amended (#3) RFP #2015-2000-2477, Medical Laboratory Testing Services - statewide

Amendment #3, dated 8/22/14, has been issued and contains a Modified Cost Proposal Form and other information.  Vendors must acknowledge receipt of this amendment as it affects how costs will be proposed.  A copy of this amendment including the Modified Cost Proposal Form is included with this notice.

Amendment #2, dated 8/20/14, has been issued and contains information with regard to determining estimated costs for required EHR lab interfaces.  A copy of this amendment is included with this notice.

Amendment #1, dated 8/12/14, has been issued and includes replacement RFP pages 34-35 and an addtional Attachment #10.  A complete copy this amendment is included with this notice.

The State of Alaska, Department of Corrections is requesting proposals for the provision of clinical laboratory services in support of inmate health care.  Medical specimens will be submitted from the Department's statewide correctional facilities for diagnostic testing and related services.

Proposals will be accepted at the mailing address below until 2:30 p.m. September 2, 2014.
