CON Approved - Long Term Care Facility - Kodiak

NOTICE OF COMMISSIONER’S DECISION TO ISSUE A CERTIFICATE OF NEED
THE DEPARTMENT OF HEALTH AND SOCIAL SERVICES

In accordance with 7 AAC 07.070, the Commissioner of the Department of Health
and Social Services has issued a Certificate of Need for the application
described below.

Project/Activity: Kodiak Island Borough proposes to construct a replacement
skilled nursing/long term care center in Kodiak Alaska.

Applicant’s Name: Kodiak Island Borough

Project/Activity Service Area: Kodiak, Alaska

Summary of Decision: In accordance with A.S. 18.07.031-111 and 7 AAC
07.010-900 Kodiak Island Borough is authorized to construct an 18,708 square
foot replacement skilled nursing/long term care center in Kodiak with a total
of 22 licensed beds.

The total cost of the project is $16,086,800 for construction, equipment, and
furnishings.

The estimated operation date is January 31, 2014 with a final completion date
of January 31, 2015.

As a condition of this Certificate of Need award, the Department must receive
and approve a transition plan.

An applicant may appeal the Commissioner’s decision by contacting Karen Lawfer
as indicated above. An appeal must be made in accordance with 7 AAC 07.080 and
must be received by the department no later than 5:00pm Thursday March 28, 2013.

Any member of the public who is substantially affected by activities authorized
by this certificate of need may initiate an administrative hearing under 7 AAC
07.082. The request must be received by Karen Lawfer as indicated above, be
made in accordance with 7 AAC 07.082, and must be received by the department no
later than 5:00pm Thursday March 28, 2013..
